*Referral from your Primary Care Physician is required for the Insurance Plans above that are followed by (*). If a referral is not available at the time of the visit, your appointment will have to be rescheduled. Please double-check on your insurance card and insurance plan carefully if referrals, deductibles or copays are needed. If you don't have any medical insurance or if you don't see your insurance plan on the list above, please call us with any questions. We accept payments only by check or cash.
